BENEFIT FACTS FOR 2012-2013

 

Any changes to your medical, dental, and vision choices need a new form completed and turned in to the district office by September 5, 2012 

 MEDICAL PLANS

 

 WEA Select Health Care Plans: Plan 5, Plan 2, Plan 3, EasyChoice  and Qualified High Deductible Health Plan (QHDHP)

Plan 1 has been discontinued by the company.  If you want Plan 2, the most similar to Plan 1, you do not have to fill out a new form.  If you want to choose a different plan you will fill out a new medical choice form located in the district office and return to Debby by September 5, 2012.

If you choose the QHDHP, you contribute your own funds into a Health Savings Account.  Please review the IRS guidelines regarding the election of a Health Savings Account as well as the guidelines and limitations regarding this type of plan.

DENTAL PLANS

Ameritas Plans: Plan A, Plan TI-LOW

Dental is now offered with four tiered pricing, the same as medical coverage.  There is a price for employee only, employee and spouse, employee and children, and employee and family coverage.  This individual choice may allow you to have more from the $768.00 state allocation dollars going toward your medical. 

ATTENTION:

NEW dental enrollment forms are required  to establish your tiered pricing.  If you do not fill out a new enrollment form, you will automatically be defaulted to the employee family pricing, the highest priced plan.

Also, Certificated staff,  Secretaries, and full time custodial/maintenance personnel have dental as per their union contracts. 

VISION

Superior and WEA VSP Plans Available

Vision plans are offered to Certificated Staff and Secretaries as per their union contracts.

Frequently Asked Questions:

WHAT IS THE STATE ALLOTMENT AMOUNT?  $768.00 per full FTE.

WHAT IS THE 3%?  This is a monthly payroll deduction based on the cost of the medical plan you choose.

WHERE DOES THAT 3% GO?  It goes to your union’s medical pooling fund.

DO I HAVE TO TAKE MEDICAL?  NO, medical is voluntary and available to those who work a minimum of 15 hours per week.

IF DENTAL AND VISION ARE OFFERED TO ME DO I HAVE TO TAKE THEM?  YES, they are considered mandatory as per your union contracts.
